How is the Day Care Service accessed?

Requests for Day Care places are taken from a variety of sources across the city, including Social Care and Health Departments, the Health Authority, or GPs, and people who wish to pay for their care privately or their family and friends.

When your referral has been accepted by Care2Care Services, we will contact you as soon as possible. You will be offered an assessment of need by one of Care2Care's Care Managers. This may be by telephone or a visit to your (your family's/carer's) home.

After the assessment of need and eligibility is confirmed, you will be offered a day care place if this service meets the assessed need. A Care2Care Care Manager will visit you at your home to agree the transport/social arrangements and start date etc.

Is there a charge for the Day Care Service?

The Day Centre is self-financing, therefore a fee is payable.

Funding is available from Sheffield City Council. To obtain funding clients will be required to undertake a financial assessment to ascertain their eligibility to contribute towards the cost of the service.

All clients receiving Council funding are required to pay a small fee as a contribution towards the cost of their lunch, which is mandatory for all clients.

We will be pleased to offer free, friendly and impartial advice on the funding options available to you.

Is help and advice available?

We will be happy to help, either over the phone, or if you prefer we can arrange to visit you in your own home to provide a 'no cost, no obligation' assessment with one of our Care Managers, together with advice concerning the funding options available.

Information about the activities, facilities and services available at the Day Care Centre can be found on the Activities, Facilities and Services page.
